To claim CME credit for this conference, you must complete this overall conference evaluation and claim credit for each session you attended.  They are all listed on the form, broken out by workshops and didactic sessions.

HOW TO LOG IN:

Use the BLUE login button on the top of this page to log into the SEMPA CME System. (If the button says Logout then you are already logged in

USERNAME:  The email address you used to register for SEMPA 360.

PASSWORD: 2024SEMPA360 (you may change it if you like)

Access the evaluation by clicking the green button below or in the "Course Content" section at the bottom of the page. You will first answer conference evaluation questions before selecting the workshops and didactic sessions you attended.  Please do not select sessions you did not attend.

When you have completed the evaluation, click "SUBMIT" and wait to be redirected to the confirmation page, where you will be able to download your certificate.  If you exit before reaching the "CONFIRM" page, you may need to resubmit your information.  

You may change your name or credentials on your certificate by clicking the GREEN "Edit Your Profile" page. If they are not already listed, you should add your credentials (PA-C, etc.) to the last name field.

Upon submission of your evaluation, you will be able to view your session by session transcript here.

This evaluation page will be live for the next 30 days, so if you prefer to submit your evaluation and claim credit after you return to your home, you may.
